00:00 - And another group, first observed, something unexpected. I, we should have expected it actually, but we didn't. and that is that, I have to back up a little bit. HHIV infection, I'm sure, as your audience knows,

00:17 - can be very, very successfully treated, with drugs that block the replication of the virus, in the body. It's, it's its ability to be transferred from one cell to the next, to the next, to the next. And we can do that very, very effectively, with modern antiretroviral therapy. one pill a day in many cases. but, as as soon as you stop therapy, the, the virus starts to come back again.

00:46 - And the reason it comes back again, is that HIV is a retrovirus. And retroviruses have the unique property that as part of their normal replication pro pathway in a cell, they have to integrate their genetic information into the cell's genetic information. And what that means is that the cell is, the viruses will always be there as long as that cell lives. Now, in HIV for reasons that we still don't, don't understand very well, a small fraction of a large,

01:19 - the large majority of cells that get infected in this way die. So they, so that they're, they're gone. And that's, that's why the disease goes away when you treat, because the cells that have were infected will die. The the, and no new cells will be infected. But a very small residue of cells, remains in which the virus, for some reason and reason we don't really understand very well, has effectively been shut off.

01:47 - So it can no longer express itself, can no longer make a new virus to spread to other cells, and the cell, and no longer kill the cell that it's in, so that cell can survive indefinitely. And it was observed by other groups, quite a, quite a long time before that the, we started working on this, that, these cells are very often a cell, called a CD four positive T cell.

02:20 - CD four T cells are cells of the immune system that are essential for, generating a correct immune response and protecting us, from, infection with bacteria, parasites, and other viruses. And the damage done to these cells is what causes aids. The loss. The loss of these cells, of this particular type of T-cell, is what leads to the symptoms of aids and inevitable, almost inevitable death if the virus is not treated.

02:52 - but if the virus is treated, those cells no longer die. But a few of them, as I said, will, can carry these, carry the, the provirus, the DNA form of the virus, as one of their genes. Basically, these pro viruses can be, in many, many different sites, in the cell genome, which is, both a hindrance to trying to, to figure out what's going on, but also can very help very much, in identifying,

03:28 - cells that, that that specific cells and, and, and identifying which cells have actually were actually originally infected to give rise to the, the cells that you see carrying the virus. And we, we, and other groups have been using a very, very nice, sensitive, assays to identify the, the DNA form. It's called the pro viruses of cells, of, in, in,

04:05 - these CD four T cells, that, have the potential to give rise to more virus even many years after they're, they're infected. when you stop therapy, they can give rise to virus. All they can give rise to a little bit of virus all the time. mo in most of the cells of virus, for reasons we don't, still don't completely understand, it's suppressed in its ability to be expressed to give virus.

04:32 - But it e every now and then, some small fraction of these cells will start to make a little bit of virus. And so there's always a little bit of virus around, even in individuals where, where the virus can no longer get from one cell to another because its ability to do so is being stopped by antiviral drugs. when you stop those drugs, that little bit of virus can rekindle the infection, like the embers basically in a,

04:58 - in a long dead fi, otherwise long dead fire. and you, you throw new fuel on it by, by, by, by stopping the, the therapy and the, that can then rekindle the infection and the, the virus. In fact, the process of infection will take off and resume really right back, right where it was before you started. And the striking thing that, my colleagues and I observed, now 10 years ago, was

05:32 - that we used an in, we used an assay that allowed us to detect not only the presence of these pro viruses in these, in these cells, and a very rare cell, less than one in a thousand cells usually carries one in these pro viruses. and, we could, but we could detect that and we could also identify exact, we could also identify exactly where it was integrated by the assay

05:57 - that we were using, which I won't go into detail, but it allowed us to see the virus, see where in the genome, which, in which of the about 3 billion base pairs of our genome, this, this integration had occurred. And we could detect one more thing. And that is, how many cells contain this provirus at exactly this site. Now, because integration is, there are very many sites, possible sites for integration of the provirus in, in the cell that the vi

06:29 - that the virus can actually be inserted at. And it's more, it's not random where they go. but we, we estimated there are more over well over 10 million sites for integration of, so, so the probability that that happens twice, in the same cell, in the same site in two different cells is extremely rare. You never see it pro essentially at one in 10 million or much, much less than probably one in 10 million. So, but we, we, we, the assay that we used, which had been devised by a, a very clever fellow

07:05 - in England, named Charles Ham, was allowed us to, not only, find the provirus, not only tell exactly where it was, but to estimate how many cells actually had the provirus at exactly that site. And from that, we could infer, given the rarity of integration it's at, at any given site, that these cells must have been the descendants of one originally infected cell. Now, CD four T cells are a, a component

07:44 - of the immune system and the job of it. One of the jobs of immune, cells is to react to, invading bacteria and, and, and pathogens other pathogens. And, and then, but another job is to, once it's, once it's dealt with a pathogen, to sit quietly, go into a quiet state, and, but remember that had had seen that pathogen, so when it shows up again, it can react very quickly and, and be be ready to,

08:20 - to take on the same pathogen again. And it does that by going through a process called clonal expansion. It's what these cells do. They, the, the, the cells all have the potential before you, before they start when they're made, to react to one particular organism or another, one particular sequence that might be found protein sequence

08:44 - that might be found in one organism or another, that be it a bacteria or cell or cell infected with a virus or, or a parasite. and very quickly increase its numbers so that it can attack the invading, as quickly as possible and get, and get rid of it. And then, and then once it's done, that go back into a quiet resting state, which most of which, most of the cells die off, but a few survive and ready to, ready to come back again.

09:15 - And you can get these, so you can get these, these cells have the property. They go through these, these cycles of this expansion, and we call it clonal expansion because it starts with a single cell. A clone is a group of cells that have been, that are derived by cell division from one, one original cell. And, and so the, the virus has the persistence of this virus. We discovered, actually hijacked has hijacked this

09:41 - clonal expansion process. and that is, what allows the virus to persist in such large numbers. although it looks very small, one in a one in a thousand cells, sounds small, but there are about, that would be about a billion cells altogether that would have a provirus in them and over your whole body. So it's really quite a large number distributed all over your body

10:09 - and just waiting for, to, find an antigen to react to. other groups have, have shown, or another group has, has shown that one of the main antigens that these cells, react to is HIV itself. Of course, HIV is invader. There is an immune response against HIV. HIV has many, adaptations. So even though there is this immune response, it's,

10:36 - it's not enough to eliminate the virus. Unlike most other viruses, which are eliminated by these kinds of immune responses, HIV can evade that. It's for reasons we're not gonna be able to get into here. And so we, we found that the, that the ability of these, the virus to form this, these to, to grow virus infected cells to grow into these large clones, is really what makes the HIV persist be able to persist for, for decades Now, we, we know examples

11:11 - of people infected 30 years or more, who've been, successfully treated and have stopped the treatment. and the virus has come right back again. And it's not only did the cell, not only the virus can sit, the, the cells containing the, the provirus can sit, but they expand. They can expand. And so that there's always more there. and, and so you really, it really is what makes the virus right now impossible to eradicate.

11:43 - Although people are trying to come up with very clever strategies to do so, nobody has come very close to even, being able to even make a dent, dent in that, resident persistent virus infected cell population. We have to keep on trying what, what people are. One of the things people are trying to do, is to, is to activate these cells. These cells are, at the time you find them, although these cells have been, have, have gone

12:15 - through phases where they have been dividing like crazy, at the, and, and it is possible to come up with therapies to, to and at, well, the dividing, dividing cells, once they start to divide the, the, the theory is they'll make virus. Although that's, that's certainly not, generally that can't be true, because they don't, the cells are able to have been able to expand without dying so that the, with, with the virus in them

And, so that, although some of them no doubt have died as a consequence of, of, of the virus becoming reactivated and being able to kill them, many of them have, many of them have not. And we don't know quite why that is either. but people have been one, one of the strategies that people have been trying for some time now is to find ways of activating the expression

13:10 - of the virus in these cells or activating the, the, the cells to, to divide specifically to divide somehow, which might then activate the virus. And so, but one has to be a little careful about doing that. You can get many kinds of, autoimmune conditions would co could come from just activating, ran, sort of randomly activating cell division of these cells. but there are some agents, people have been trying that to, reactivate virus expression
